Welcome to the FY23 Wellness Program Year

The FY23 Wellness Program year is officially here! Like last year, you will need to complete the online health risk assessment, a biometric screening,
and earn 5 additional points (a total of 7 points) by June 30, 2023 to qualify for
the $600 wellness rebate ($1200 if your spouse/FIP completes the requirements as well). If you earn a total of 12 points by the program deadline, you will also qualify for
the high point raffle.
There have been a few changes to the program since last year that I wanted to make
you aware of.
WHAT'S CHANGED
- The "4 Online Classes/Webinars" activity has been replaced with webinar quizzes. Instead of having to watch 4 webinars to earn one point (for a total of 4 points), watch a webinar (found under Quicklinks on your PreventionCloud dashboard) and then take the corresponding quiz.
- The quizzes can be found on the left-hand side of your PreventionCloud dashboard under "Quiz."
- Each quiz is worth 0.5 points for an activity total of 2 points.
- You can now earn points for more preventative healthcare screenings. When you login to your PreventionCloud dashboard, click on "Plans" on the left-hand side of your dashboard. From the drop-down menu that appears, select "University of Alaska Preventative Health Screenings." This will show you the health screenings that are recommended for your age and gender, including your dental, vision, and annual wellness exams.
- Your age/gender preventative screening form can be found in your physician visit packet, the same form that is used for your biometric screening.
- To submit your age/gender preventative screening forms, click on "Health Forms" on the left-hand side of your PreventionCloud dashboard and choose "Submit Forms." From the drop-down menu provided, choose "Age/Gender Preventative Screenings." Upload your form and click "Submit."
- You can earn up to 5 points for this activity.
- The COVID Vaccine Attestation and the Adult Immunization Activity have now been rolled into 1 activity that is now worth 2 points. The COVID Vaccine Attestations that were submitted last year have been rolled over into this year so you do not have to resubmit your attestation. However, you can still earn a point for any other vaccine you may receive this year.
HOW TO LOG IN TO PREVENTIONCLOUD
- Single Sign On is available for employees. To access Single Sign On, please go to the University of Alaska benefits page at: .
- Spouses/Financially Interdependent Partners (FIPs) can only login using their PreventionCloud credentials.
- Go to
- Your username is your FIRSTNAMELASTNAMEBirthyear
- Your password is your birthdate in the format MMDDYYYY
- For example: John Smith with the birthday September 9, 1965 would have the username JOHNSMITH1965 and the password 09091965.
- After your initial login, you'll be prompted to change your password. You may choose whatever password you wish.
